        • Feltin Home Visit Program

          • This program serves our patients who are enrolled in Community Care Cooperative (C3) Medicare Shared Savings Program (MSSP) and provides:

            • Regular ongoing home visits

            • Routine follow up and transitional care visits posthospitalization

            • Same-day triage and visits as needed for new issues

            • Refills, prescriptions, medication reconciliation, DME orders

            • Vaccinations, screenings, collection of lab specimens

            • Robust care coordination with other care team providers, family, and community supports

            • Care management and social support services

        • GLFHC Home Visit Program

          • This is a compressive, individualized, home health care program run by Tanya Campus MD, that is predominately for the elderly.

          • Program space is very limited, but a good option for someone who is not enrolled in Community Care Cooperative (C3) and needs individualized care.

        • Home Health Aides (HHA) and Personal Care Aide (PCA)

          • A personal care aides (PCA) focus on non-medical care, while a home health aides (HHA) provide basic medical care.

        • GLFHC Remote Patient Monitoring Program
