        Take note of the USPSTF requirements low-dose CT lung cancer screening (CMS follows these rules):

         

        Population

        Adults aged 50 to 77 years who have a 20 pack-year smoking history and currently smoke or have quit within the past 15 years.

         

        Recommendation:

        The USPSTF recommends annual screening for lung cancer with low-dose computed tomography (LDCT) in adults aged 50 to 80 years (NOTE: LGH restricts this screening to age up to 77) who have a 20 pack-year smoking history and currently smoke or have quit within the past 15 years. Screening should be discontinued once a person has not smoked for 15 years or develops a health problem that substantially limits life expectancy or the ability or willingness to have curative lung surgery.

        If your patient meets these criteria, then you can order low-dose CT lung cancer screening using the order set:

         

         SCRN - Lung Cancer Screening - Low Dose CT Referral, LDCT

         

        That order has several required fields that must be filled in correctly in order for the patient to qualify. When you add the order set the order will appear with red bars indicating missing required fields:

         

        These are the required fields:

         

         Has patient had Lung CA Screening Counseling and Share decision making visit as required by CMS?

        • Yes

        • No - If you enter no, then the patient will not qualify for this study.

        Current smoker or one who has quit smoking within the last 15 years

        • Yes

        • No - If you enter no, then the patient will not qualify for this study.

        Packs per day

        • Enter the number of packs per day smoked (if the patient has stopped smoking, it is OK to enter an average number). 

        Tobacco smoking history of at least 20 pack-years (calculated as packs_per_day x number_years_smoking) 

        • Yes

        • No - If you enter no, then the patient will not qualify for this study.

        Height

        • Enter the patient’s height in either inches or feet’inches” (unfortuantely we cannot pull it into the order).

        Weight (lbs)

        • Enter the patient’s weight in pounds (unfortuantely we cannot pull it into the order).

        Patient has no signs / symptoms of lung cancer

        • Yes 

        • No - If you enter no, then the patient will not qualify for this study.

         

        Please take note of the correct answers to qualify are all “Yes”, and most importantly, the patient will be asked these questions again at the time they present to radiology. If they do not answer the questions the same way, they will be disqualified, so please make sure that you and the patient agree on the answers.
